CLEVELAND, Ohio -- Oak Street Health, a network of primary care centers for adults on Medicare, has opened its fourth care center in Cleveland.
The center at 6410 Broadway Ave. is Oak Street Health’s seventh center in Ohio, joining centers in Dayton and Youngstown.
“As our company continues its mission to rebuild healthcare as it should be, we are ready to welcome new patients in Cleveland and help improve their quality of life,” said Tamara Jurgenson, chief growth officer of Oak Street Health, in a written statement.
